A dental implant is the product of scientific research by dentists and researchers alike that mimics actual tooth in every detail. The only difference, the non-existence of the root, nerves and pulp and other parts that are essential for the function of the natural teeth. The implant itself is made of titanium, an expensive metal that is also used as frames for bridges and medical implants. This is the preferred material for it has the lowest chance of being rejected by the body’s immune system which normally treats it as inert thus does not attack it causing an infection. The implant is also embedded (well screwed in real terms) into the bone of the jaw structure to give it similar strength to natural teeth which is cemented by natural glue to the said bone making it rigid. The base and root are made of high strength ceramic that can withstand the bite force of natural teeth if not more making them strong to last.

This is the case in many elderly people or for those who have had some trauma to the jaws which may leave them with permanent disfigurement of the underlying bone structure. Titanium implants are used in a process called denture stabilization to allow a set of standard dentures to sit on something durable and tough enough for daily use. This procedure has changed the lives of many who have had to do with ill fitting dentures that either fall off or create sores due to repeated rubbing with the gums which is not only painful but not acceptable. New titanium implant technologies have allowed the creation of seemingly natural teeth from porcelain or other composite materials, which are not only more durable than normal teeth but also allows resumption of normal life allowing food intake without previous inhibitions.

The dental implantation procedure is actually a simple one, comprised of only two steps:

1. Pre-operation
The jaw is prepared for in procedure, which involves drilling at the edentulous jaw sites – sites where there is no tooth – for the titanium screw, which is part of the dental implant. The hole, which is very small, must be drilled with great care so as to not damage any nerves in the jaw.

2. Implantation proper.
Just before the implant is screwed into place, the pilot hole is slowly widened so that the dental implant will fit into it better. Once the implant itself has been screwed in place, a protective cover is placed on top of the screw to allow osseointegration – the anchoring of the dental implant to the jaw.

Clinical mobility is the failure of the normal mechanics which ensures osseointegration of an implant into the bone preventing proper stability thus allowing the implant and other structures it supports to move around. This can cause more damage for the soft tissue becomes inflamed and can even have an infection spreading into the jawbone itself defeating the procedure that has been done. Another problem would be occlusal overload by which too much implants has weakened the jaw whereby preventing further improvements to be done.

The Nobel system has been getting more and more criticism from general practitioners who have steadily been sending in reports that failures are on the rise and that the product has some design flaws that should be corrected. The Nobel Company has responded with statements and notes that failure is more on the side of the implantologist and not the product but dentists refute that specifying the fact that they have followed all standard protocol with respect to the implantation of their products. We will be following future developments on this issue for there are hundreds of thousands of people all over the world with this type of implant that may be at risk of implant failure.

The timing is the most critical factor and the implant should be inserted as soon as the tooth is pulled out improving chances of biointegration of the selected implant. Biointegration is the process by which implants (which are usually made of titanium) are placed into the bone which then forms a sort of plug around it. Using an electron microscope, researchers have found that the stability of the implant is only as much as the amount of bone that has grown between the space of the extracted tooth and the implant. The microscopic analyses of implant procedures also shows that the implant is not totally embraced by the bony tissue rather than a gap which is filled by the oxidized titanium (which is inert and does not affect the human body) which causes the adhesion strength required for success.

So advances with cone beam imaging was a welcome sight indeed, allowing better treatment for previously general problems. It gave dentists the ability to prescribe the best personalized treatment for each and every patient depending on their needs without the tooth-pulling of yesterday.
It lessens the risk of too much radiation compared to CT scans and it allows more detailed images of the teeth and underlying structures. Success and failure are balanced out with proper imaging and treatment thanks to better views of our pearly whites.

And guess what? It may soon come to an end for researchers may have cracked several leads on how to stop the process from beginning in the first place. Several types of bacteria live on human teeth and for them to do so, they need chemicals that allows them to attach themselves to the hard surface of our teeth.
Take these enzymes away and bacteria looses it’s ability to grab a foothold preventing tooth decay instantly.
Better oral care products that contain compounds that mimic human saliva) that cleanses the mouth better. All of these may soon come to a tube of toothpaste or gum near you, leaving us smiling from birth till old age with all our “Pearly whites”!
